Self-Sustaining Application
Confidential Financial Statement
The above forms are reviewed by designated representative(s) of the Elks National Home. A $100 per person non-refundable Application Processing Fee is required to be paid with the Application. Application forms are available by emailing, enhome@elkshome.org; telephoning, (540)586-8232 or 1-800-552-4140; or writing to Elks National Home, 931 Ashland Ave, Bedford, VA, 24523.
An approved application can be held for up to 60 days for the resident's arrival. In addition to the monthly service fee, you are also required to pay an occupancy right fee upon your arrival. See below for details.
Insurance, stocks, etc., cannot be accepted in lieu of cash.
The minimum occupancy right fee is non-interest bearing to
the resident. If more than the $10,000 (or $5,000 for Building H) minimum
is required, then the excess amount would be placed in a security deposit in
trust with the Home and this security deposit would be interest bearing for
the resident.
The minimum occupancy right fee is ninety percent (90%)
refundable if residency is terminated within the first year for any reason
other than death. Each year thereafter, the fee decreases an additional ten
percent (10%) of the original amount until it reaches zero. Deductions will
be made from any amount to be refunded for any balances due the Home. In
the event of the resident’s death at any time after arrival, the Home
immediately earns any remaining balance and there is no refund.
If a resident is required to pay a security deposit in addition to the minimum occupancy right fee, this excess amount is refundable to the resident or his estate upon the resident’s death or departure from the Home. Deductions will be made from the refund for any expenses incurred or paid for the resident.

Health Care
Although we cannot accept those already in
need of assistance or supervision, we have a section for residents who become in
need of assisted living after they have passed a three month probationary period.
Catered Care is also available. This is limited assistance given to a resident
in his/her own room.
A physician is available to residents during scheduled office hours at the Home. The resident is responsible for all
charges, which will be billed by the physician's office. The Home will
bill for any medicines or supplies received. There is a charge of $65.00 per day
for inpatient care in the Special Care (assisted living) unit in addition to the
monthly maintenance charge. Basic Catered Care is $15.00 per day.
Bedford County Memorial Hospital is located 1.5 miles from the Home for residents who need hospital care.
Our medical records department will assist you in filing your Medicare and other insurance claims for outside medical treatment.
Finances
In order to qualify as a self-sustaining resident, your income and assets must be sufficient for you to be able to pay the monthly service fee, provide you with spending money, and meet any other expenses you may incur for supplemental insurance, special care, outside medical care, medicines, etc. The lodge accepts no financial responsibility for a self-sustaining resident. However, a portion of the application must be completed by the lodge.
A SELF-SUSTAINING RESIDENT DOES NOT HAVE TO TURN THEIR INCOME AND ASSETS OVER TO THE HOME
Those who do not have sufficient income and/or assets to meet the self-sustaining requirements may request a partial-maintenance application, which requires the lodge to accept financial responsibility for expenses the resident cannot pay. More details on this procedure are available from the Home. You may call our toll free number during regular business hours at 1-800-552-4140.
